Patient Navigation in Lahaina

 
 

We are honored to share that Pacific Cancer Foundation is now offering in-person patient navigation support in Lahaina every Wednesday for the month of February.

For individuals and families impacted by cancer, the path forward can feel overwhelming. Questions come quickly. Appointments multiply. Paperwork stacks up. Decisions feel urgent and deeply personal.

You do not have to navigate it alone.

Each Wednesday in February from 9:00 am to 12:00 pm, our Patient Care Coordinator, Syrah Vergara, is available at the Hawai‘i State Department of Health Ka 'Oihana Olakino in Lahaina to meet one-on-one with community members who need support.

This service is free to anyone in Maui County impacted by cancer.

Whether you are newly diagnosed, in active treatment, a survivor, or caring for someone you love, we are here to help reduce overwhelm and provide steady guidance.

Support may include:

• Care coordination and resource connections
• Transportation and appointment support
• Lodging support when traveling for treatment
• Guidance on financial assistance, when eligible
• Referrals to trusted support programs and community partners

Sometimes what people need most is simply a place to ask questions and be heard. Other times it is help making a phone call, completing an application, or coordinating services across providers. Every situation is different. Every person’s experience matters.

Offering patient navigation in Lahaina this February reflects our ongoing commitment to ensuring that individuals and families across Maui County have access to compassionate, personalized support close to home. We are grateful for the opportunity to be present in the West Maui community in this way.
